I HAVE to go through this app to get GLP's prescribed. It took over 2 months to get an appointment to see the Doctor, time my own Doctor could have had prescribed for me to be using the medication. Once I saw the doctor, I was not able to reschedule for a little over 6 weeks. My own Doctor would have seen me in 3 weeks so they can up me to the next level of dosage to start the journey to help my prediabetes. The initial dosage is apparently just to get your body used to taking this medication. So the big thing is they are understaffed and cannot "see" patients in a reasonable amount of time. Other than that, I guess I'm getting my meds. The Doctor I saw was nice and seems to know a lot about GLP's, that's positive. We will see how things go moving forward.

One of the most important facets of Form is to interface with the insurance company to make sure the medication is covered, but I’m having to call and make sure the medication is covered every time because the prior authorization wasn’t met or my health care renewal has a different form to send in, or some other issue. Roughly every other time I go to get the medication there is a problem. I’ve had to pay out of pocket for zepbound on several occasions due to insurance issues and it’s exhausting and expensive. A huge part of why I’m paying for Form is to take care of the insurance side and that’s just not happening. The zoom doctor visits have increasingly long wait times as well. I wouldn’t recommend the service for these reasons and am researching other providers for myself. After receiving the response from Form which is essentially contact the info email, I revised the stars from 3 to 2. This is because I have tried to connect with the company about this issue on several occasions. Their response to this review perfectly encapsulates the customer service you can expect from the company - offhand, rote, and unhelpful.

Very intrusive service. You have to fill a lot of information before you start the service, wait for a month, and instead of having a session with an endocrinologist to understand the case they set an appointment with a PM and a dietitian. If you dont do the dietitian sessions then they wont prescribe the medication

The primary reason for FormHealth, in my opinion, is to provide prescriptions for Zepbound or Mounjaro to Lilly. There is not enough attention given to expediting this very thing: prescription submission. It is unacceptable to wait 72 hrs for an answer about a prescription when the drug — if prescribed — must arrive by delivery requiring another 48-72 hours. In the best case, it is a 6-day wait for a 28-day supply. That means that a refill request should be in process sooner than a week before; that is week 2.5 in a 4-week supply. I suffered a 10-day delay recently; I had to call Lilly Direct 5 times: they had no prescription; it hadn’t been approved; it was ‘in process’ — for 5 days?! I was leaving on vacation; my doctor knew this from weeks before. No Rx was submitted. I sent a text to the MD asking for it; that msg was sent immediately to one of your ‘agents’ — my doctor never responded — who unhelpfully gave me contact info for Photon. Of course they don’t write prescriptions, they had none; my doctor was on vacation. I had already discussed with her this 5-week period. The covering MD didn’t know my history, wrote for one 4-week supply. This started processing after another mandatory 72 hour review. I needed 2 refills to cover 5 weeks. I needed another prescription; another MD message text, another Photon referral; another LillyDirect call. In this one, a warning that I would not get another early fill for a full year: limited to 2 times in 365 days. This will not fit my life, and in my opinion is irrational & incomprehensible. Tirzepatide (Zepbound) is a maintenance medication similar to hypertension. It is not a Schedule II narcotic. I am a retired hospital pharmacist and I will not restrict travel plans to New Zealand in 5 months; to Japan 4 months after that. I will begin saving overfill in used vials as possible emergency back-up caused by FormHealth delays which will subject me to some level of risk in reuse. I will definitely seek out another more reliable provider so that I can travel and maintain my health. There will be many others just like me. FormHealth should properly address these issues: prioritize prompt prescription submission, knowledgeable people who can remedy obstructions and most of all facilitate; it would do well to rethink the basis of its existence, the purpose of these medications and align it with refill policies. Zepbound is not fentanyl. Educate personnel about what needing a prescription means. Photon does not write prescriptions. I was sent to them every single time I texted I needed one…. follow up: I read your response for thoughtful action. I want you to also know this: that I am presently in France in the 2nd week of my 5 week vacation (after finally getting the 2 x 28-day refills I needed). And today — today(!!), ~2.5 weeks after receiving the last of my 2 refills in 365 days vacation rule — I got a text message from Lilly: that my next refill is ready for approval. What? I have 2 refills, 8 weeks worth on hand. Isn’t that somewhere in your records when I was questioned with dates, vials remaining on my 5th call to Lilly Direct?? That Lilly now has another 28-day refill immediately waiting for my approval when it just delivered a 56-day supply? If I were a drug addict, hallelujah! More drug to abuse. But this is a serious maintenance medication; I’m keeping track; I don’t need a refill. FormHealth needs serious attention here; why are you approving another refill?? This reflects sloppy, chaotic operations and frankly, makes the vacation rule look absolutely ludicrous. Is this what you’re encouraging? That If I were home, to get that next refill to squirrel away for my New Zealand or Japan trips? I can’t. I’m on vacation and you’re not paying any attention.

The Form Health approach is a holistic care plan that employs boarded medical professionals and registered dietitians. It is comprehensive and thorough. In addition their integration with Medicare has been easy from the patient point of view. I am extremely satisfied with the level of service, the speed of service, and the cost.

Form Health, I am lodging a formal complaint regarding your failure to provide my prescribed GLP-1 medication. Despite clearing prior authorization and meeting expedited exception criteria, my care is stalled. Your onboarding is incompetent, and multiple staff have failed to provide promised updates. Release my medication immediately. If not resolved by tomorrow, I will escalate to my HR and regulators.

I signed up and was denied participation. They said I was ineligible based on my medical records. I never had a visit, but my insurance was billed for a telehealth appointment. They are charging for a service they did not complete.

I wanted to like Form Health so badly, but my experience was frustrating from the start. My initial appointments with both the doctor and nutritionist were overbooked and required weeks of waiting. The bigger issue came after Form Health supposedly screened my insurance. Once billing began, the claim was rejected, and I was then told I would need to pay $149/month for a new subscription I had not been clearly told about. What bothered me most was that Form Health refused to take responsibility for the insurance oversight, despite the fact that I relied on their screening process before moving forward. Based on my experience, this is not a company I would trust again during a weight loss journey.

The enrollment team has been somewhat unresponsive, often taking several days to reply. When they do respond, the messages are quite brief and do not include polite phrasing such as “please,” which has been a bit disappointing.

I purchased your GLP-1 membership two months ago, and to this day, I have not received a refund—which I requested because of your terms and conditions, and because you never respond to emails. The lack of customer service is unbelievable. I have messages from them saying they would call me, but they never do.

Forces you to do everything on their app, which has no accessibility features. At the very least, if they're going to be in cahoots with insurance companies to force people to use their services or no longer cover medications, then make a more accessible option for people with disabilities.

These new tech first companies do not have everything figured out yet, so be careful with your expectations. Sometimes they can fix a problem but do not have a way to communicate. So you need to enable them with a telephone! They do not see the need for telephones in modern business and that kept me waiting an extra week for meds! That's not care, that's negligence!

I began with Form Health in September. I was quickly assigned a clinician and dietitian. They were both present and helpful for all appointments. They were completely invested in my health and wellbeing. Early last month I was switched to a new clinician and he has been just as wonderful . The patient support is excellent and Staci Mullis responds quickly and gets me any assistance I need . Wonderful company !
